We Apply the Four C’s Framework

Creating awareness and understanding of the debriefing process and minimising the fear of participation is fundamental to its success.

At One Team Risk Management Solutions we deploy the independent, zero-blame 4 C’s  Framework, focussing on:

  1. The Circumstances
  2. The Causes
  3. The Consequences
  4. The Corrective Actions required.

It is a collaborative, safe-environment approach to understanding successes and opportunities following an event or an incident. The 4C’s is a forward looking framework, i.e., it looks at the past, only to improve the future.

We Address the Circumstances, Causes, Consequences and Corrective Actions.

We assist in creating a clear, collective understanding of the circumstances that create the need for a debrief or an after-incident review. We identify what was supposed to happen (what was planned or expected), and  get agreement on what actually happened (divergence from planned outcomes and expectations).

We adopt a non-personalised, no blame approach to exploration of the causes, that is, why unexpected outcomes materialised or why planned activities were not as effective as envisaged (how did the divergence from planned outcomes and expectations come about).

We assist in identifying and achieving collective understanding of the unintended consequences that materialised, including who or what has been, (or could have been by virtue of a near miss) affected by the actions or outcome.

We get agreement on the corrective actions (opportunities for improvement) required to be implemented going forward, what individual stakeholders will collectively contribute to ensure effective implementation and continuous improvement, and how this will be monitored and evaluated.

We Create a Safe Space to Gather the Information.

The debriefing or after-incident review processes should be safe, professional, positive opportunities to facilitate learning and improvement for all, however, this is not how they are generally perceived and therefore, their effectiveness has tended to be underwhelming.

Gathering the relevant information associated with identifying and understanding the circumstances, causes, consequences and necessary corrective actions associated with near perfect or unplanned, imperfect outcomes is where the difficulty has historically arisen. Such a reflective, information-seeking environment needs to be based on a safe, structured, independent, professional, non-personal, inclusive approach.

At One Team Risk Management Solutions we provide such a safe service, whether it be a 5 to 15 minute hot debrief or a 2 day After-Incident Review we apply the 4 C’s framework, clearly outlining the rules of engagement and effectively managing the audience to safely deliver the intended outcomes.
